Hospital
- All medical insurance cards, including Medicare and Medicaid
- Valid picture ID
- List of medications, including dosages
- A copy of your advance directives
- Any of the following items that you use:
- Eyeglasses
- Contact lenses
- Hearing aids
- Dentures
- Wheelchair
- Braces
- Prostheses
- Clothing and personal hygiene items:
- Slacks or sweatpants
- Shirts/blouses
- Sweaters or sweatshirts
- Seasonal outdoor apparel
- Socks
- Comfortable rubber-soled shoes or sneakers
- Sleepwear (pajamas, robe, slippers)
- Special soaps/lotions
- Toothbrush/toothpaste
- Razor/shaving cream
- Deodorant
- Cosmetics
- Brush/comb
*Do not bring valuables such as jewelry and cash to the hospital.
Outpatient
- All medical insurance cards, including Medicare and Medicaid
- Valid picture ID
- Your order for therapy
- Your insurance referral or authorization should your policy require you to obtain one prior to seeing a specialist
*If your visit is covered by workers compensation, please bring your claim number, the insurance carrier’s billing address and your job description.
